If you plan on getting a flu shot sometime within this month, then you can use this mail-in rebate form to get $5 back. You have to get your shot before 11/30 and then mail in the form and original receipt before 12/30. There is a limit of 1 per name, address, or household though. This is still $5 that you will get back!
